In the form you will enter in the basic information needed to start the new 4K enrollment process.
Students must be 4 years old on or before September 1st, 2024.
Before you begin your online registration for your new student, please gather the following:
Household information - address along with *Proof of Residency for new student enrollments (unless applying under the open enrollment
application) and phone numbers for all adult household members
Legal Parent/Guardian information - work and cell phone numbers, email addresses
Student information - demographic information, *Original Birth Certificate, *Immunization Record, health/medication information, as well as
previous school report card or transcript as appropriate
Emergency contact - hospital, doctor, dentist and emergency contact phone numbers and addresses
Request for Records - address and phone of previous school unless never before attended school
The documents marked with an *(asterisk) above are required documents to enroll your student(s) and will need to be brought in to any Elementary School Office before the registration is finalized. These documents must be submitted before your child will be enrolled.
Proof of Residency - Need one of the accepted documents:
- Current Utility Bill (within 45 days) - gas, electric, or water with name, address and current service address
- Current Cell Phone Bill (within 45 days) - with your name and address
- Current Tax bill for your home - with your name and address
- Current Lease Agreement - current address, names of all parties residing at the address, effective date and all parties signatures
Other Documents needed:
Original Birth Certificate - if you are in need of an original birth certificate for your student you may request a birth certificate from the
Wisconsin Department of Health Services by calling them at (608) 266-1371 or an completing an application on their website at Wisconsin

STEP 1 - 4K Pre-Registration Form
Once the 4K Pre-Registration Form is complete please go to any G-E-T Elementary School.
From there the office staff will collect your paperwork needed to complete the enrollment process online. Please remember to bring a copy of the students Immunization Record, Original Birth Certificate, Proof of Residency (see list above in Enrollment Requirements),
After all documentation is provided, the district will accept your student as our New Student into G-E-T School District. You will then be enrolled into your Parent Portal account. The sign in information will be sent to the email provided. This may take 1-3 days depending on day of the week submitted.

Once the information is received you will need to sign into the Parent Portal and complete all the required forms in the Enrollment Tab to New Student Enrollment and any optional forms in the General section needed. (Enrollment Tab is in the Forms section in the Parent Portal under student name)
It is important that all forms are filled out in New Student Enrollment section once you are into PowerSchool Parent Portal.
